RAD Dance is a year-round program. Tuition is billed on the 1st of each month and continues unless you withdraw.

• Covers administrative costs
• Includes a RAD Dance T-shirt for your dancer

• No refunds or prorations for missed classes
• Prorated tuition if you start mid-month
• Credits available for absences up to 2 weeks (must have written notice [to info@raddancestudio.com] at least 1 week prior to class of absence for it to be applicable as RAD credit)
• Customers with 3 consecutive absences and no written notice (to info@raddancestudio.com) may be automatically withdrawn by RAD inorder to open up their spot

• I understand that my account will be charged for tuition on the first of the month (reoccurring). Tuition can either be submitted via Automatic Charge to Credit Card or Debit Card. Cash and check payments will not be accepted for tuition
• It is the family's responsibility to update or manage Credit or Debit Card on File changes by contacting RAD office via phone or email at info@raddancestudio.com. RAD Class schedule and tuition fees will be mirrored online via Zoom in the event of any unforeseen extended studio closures
• If I decide to withdraw enrollment, I agree to fill out the appropriate paperwork (Request for Class Adjustment/Class Drop Form) and follow correct procedures. I understand that there are NO CASH OR ACCOUNT REFUNDS, but I may receive STUDIO CREDIT if any credit remains (good for one year after drop date) minus a $25 drop processing fee
• Declined credit card transactions must be resolved within 5 business days. A $25 late fee will be added to declined tuition fee on 6th business day. I UNDERSTAND THAT IF A CLASS HAS 8 OR FEWER STUDENTS, RAD MAY REDIRECT/CANCEL CLASS DUE TO LOW ENROLLMENT.

• While attending classes or events affiliated with RAD Dance Studio or RAD Dance Afterschool Program, because movement is involved there is always the possibility of injury/illness. RAD will provide responsible care for my child, but, if some injury/illness should occur, I, the undersigned, agree not to hold RAD, the owners, any of the chaperones, or any of its staff responsible. I further agree to indemnify and hold harmless RAD, and its agents and employees from claim, losses, injuries, and damages of any nature whatsoever incurred. I further authorize the agents of RAD to obtain any and all necessary emergency health care, in my stead, for my child. By checking this box you are confirming that you have provided RAD with a list of any allergies, medications, or previous injuries that may be pertinent to your child's participation in dance.
